Your first appointment gives our team an opportunity to learn about your symptoms, goals, and activity level. The more information you can share about your concern, the better!
In most cases, we will also perform a physical assessment in which we evaluate your movement, strength, balance, and mobility. For that reason, please wear comfortable clothing that allows easy movement. Athletic wear, including athletic shoes, is ideal
To help your visit run smoothly, please bring:
- A photo ID
- Your insurance card, if applicable
- Any referral paperwork, if provided by your healthcare provider
- Relevant imaging reports or medical records related to your condition
- A list of current medications, if needed
- Completed paperwork, if possible, as that helps maximize the time you can spend with your physical therapist during your visit
